Photography Agreement

Participants are advised that photographs or videotape of participants may be used in publications, websites, or other material produced by Our Lady of Hope, Our Lady of Hope Retreat Center, or Camp Chatawa, however, participants will not be identified without specific written consent. Participants who do not wish to be photographed or filmed should notify the office of Our Lady of Hope in writing.

I have read and agree with Our Lady of Hope Catholic Retreat Center's Photography Agreement.

Accident Waiver and Release of Liability

I understand that participation in the event or activity for which I am paying for could include actions or tasks which might be hazardous to the Retreatant/Participant named above. By checking the box below, I assume any risk of harm or injury which might occur to the participant due to his/her/my participation in the event/activity/retreat. I also assume any risk associated with the contracting of Covid-19 related illness during or after my stay at the event/activity/retreat. I release and hold harmless Our Lady of Hope Catholic Retreat Center/Camp Chatawa, Diocese of Jackson, Pike County, Board Members, Staff Members, Volunteers, their families, nor any person affiliated with the named organizations responsible or liable for any and all costs and damages which might arise from participation in the above event, activity, or retreat. If the participant is a minor, I agree that the minor has my consent to participate in the event. I further provide my consent for the organization or business named above to seek emergency treatment for the minor if necessary, I agree to accept financial responsibility for the cost related to this emergency treatment.
